Modules with no explicit header should be kept headerless

Description

Currently haskell-src-exts inserts a default "Main" module header, a legacy of haskell-src. This is a wart for many reasons, and obviously won't fit with roundtripping.

Headerless modules are now kept headerless when using the more precise annotated AST. Case closed.
